Injured worker claim history profile

Injured workers can view a list of all claims associated with their
own Social Security number. Injured worker designees, representatives and
managed care organizations (MCOs) can view a list of all claims associated
with a given Social Security number but are limited to viewing only the
claims with which they are associated. Medical providers can view all claims
associated with any given Social Security number, regardless of their
association.

They can view the following information for each claim:
- Date of injury;
- Injured worker;
- Benefit type;
- Claim status;
- Employer name;
- Employer doing business as name;
- Employer policy number;
- MCO name.

Info menu to view additional information.
