Industry-Specific Safety Program participants who selected on-site consultation/training as one of their loss-prevention activities must complete this survey. The person(s) in the organization in attendance during the consultation should complete this form. It should only take about five minutes to complete. The SH-29 contains six categories for you to evaluate. You will be asked to rate your company's experience using the scale below. Rating scale: Excellent; Good; Fair; Poor Rate your company's experience as honestly as possible. There also is a place for you to provide additional comments. |
